The Smith and Wesson Model 500 is seen in Madness Combat 7: Consternation, Madness Combat 8: Inundation and Madness: Project Nexus. It is one of Jesus' signature weapons along with his binary sword and his TAC-50. It is very powerful and can kill a normal-sized enemy with a single shot.

It is the most powerful but lowest capacity pistol in Madness: Project Nexus (Classic).

The S&W M500 later returns as the Nevadean-500 Magnum Longbarrel (N-500 L for short) in MADNESS: Project Nexus notably apart of Christoff's loadouts much like the main series. The Long barrel version rarely appears in the campaign otherwise but the snub version can sometimes be found in loot containers and in the hands of enemies. He also uses this weapon during his boss fight. In Arena Mode, the Player can acquire this weapon from Quartermaster Bert once both Slaughter Time and Haunting of Nevada House are completed.

Trivia

• Throughout the course of its use in Madness Inundation, no shells are seen ejected from Jesus' S&W 500. This is possibly due to a design oversight.
  • However, the shells are seen being discarded in Madness: Project Nexus (Classic) when the gun is reloaded.
