The USAS-12 is an automatic shotgun designed as a combat shotgun manufactured in South Korea during the 1980s. Because of its enlarged box magazine and automatic fire, it is the most powerful shotgun in Madness: Project Nexus (Classic). Due to this, the USAS-12 is not purchasable in Arena Mode until the player reaches level 20. It also appears in Incident: 1000A, however with a shorter barrel. It is in the hands of an Agent to fight against Jesus, but the latter used telekinesis to shoot all the agents in the room with it. It made its first canon appearance in Madness Combat 9.5 in the hands of a Soldat. The soldat fires multiple semi-auto shots at Hank, the only shot connecting hitting a corpse behind him.
