The minigun is a rotary barrel machine gun derived from the Gatling gun. In Madness Inundation, the Auditor used a minigun to combine sniper-rifle accuracy aim with the gun's destructive power against Jesus but misses with every shot. The minigun also appeared at the end of Incident: 110A but wasn't used until after the credits screen suddenly cuts off when Jesus used it against a mob of oncoming agents.

In Madness Interactive, miniguns have unlimited ammo, high power and an incredible rate of fire. The minigun can only be used in the Madness Interactive Story Mode if a cheat code is used.

In Madness: Project Nexus (Classic), all G03LM Mk2s, in both Story Mode and Arena Combat Mode, have one as a secondary weapon. Like the mega hammer, the minigun is one of the few unusable weapons in the game.

The Minigun reappears in MADNESS: Project Nexus as one of the rarest yet most expensive weapons in the game. In Story Mode, The Blackguard uses it upon having two corpus blocks left. In Arena Mode it can be bought on average for around 1.6 Million from Chef Pava, making it the most expensive weapon in the game (though it is reduced in cost upon the completion of Mandatus.) It's however can be used by MK.2 G03LMs during Nexus Core Facility and will be dropped upon their death.
